What's the most embarrassing clothing malfunction you have had?

Swimming wearing a pair of "cut offs" old jeans that we used to cut off above the knee instead of buying shorts. I dove into the creek and the pants came off. When I surfaced, one of my buddies was waving them around over his head and everybody was laughing. Then the game of "keep away" ensued and it took awhile to get them back on. 4/21/26
