If I meet someone in my professional capacity they are Mr., Mrs. or Ms. until they tell me otherwise, and I expect to be addressed formally when I am a customer or client unless or until I tell them otherwise. Outside of work I can only think of two people, both elderly ladies (both now deceased) that I have known as a "proper" adult and always addressed formally. I didn't know Mrs. W's first name, I did know Mrs. B's but I would never have dared to use it!
