Really good question! My Father-in-Law is a doctor. He explained it very well. Depending on the drug, some actually become toxic, some have expiration dates for marketing and to make new sales. If you are not a doctor, chemist, or pharmacist, follow the guidelines on the container. Many antibiotics can become toxic after a few years, some other drugs lose their effectiveness. Become friends with your pharmacist or doctor, and they will give you the truth about individual drugs.

They work for a year or so after the expiration. But they do start to lose potency. Just as a opened bottle can lose potency before the expiration compared to a sealed bottle.
