It goes inside the quote marks. And you put a comma after 'said' And there is a full stop of the paragraph unless he is 'doing' something. The correct way to do it singular is: He said, "What are you doing?" The correct way to do it with continuation is: He said, "What are you doing?" Then he reached and picked up the knife.
