You said thoroughly cleaned, so why not? I guess the idea is a bit of a turnoff, but I wouldn't say unhygenic.

If the toothbrush hasn't been cleaned, yes, it is very unhygenic. The brush needs to be cleaned properly. The most popular way to clean it is by boiling it.

I would not use the used toothbrush of anybody that I would not tongue kiss. If you already are exchanging body fluids then hygene is really a mute point anyway.

Personally I think it's a bit disgusting, if you're in a relationship with that person then it doesn't matter anyway does it!

Our toothbrushes are kept in a glass, in a cupboard, just above the toilet. I am always dropping one into the toilet bowl. A quick rinse under the tap gets rid of all the germs, right ?

Depends on what you mean by "thoroughly cleaned" if you are including all germs as in you boiled it, or soaked it in bleach, alcohol, or vinegar then its fine. Other than that its pretty much a judgement call on whether its cleaner than your mouth.
