  • It depends on the type of health insurance you have. When managed care first became widespread in the U.S., most HMOs said that primary care doctors needed to serve as "gatekeepers" to specialists. However, there was such an incredible backlash that they backed off. Now the majority of managed care plans allow you to go to a specialist without a referral, but only if the specialist is "in network."
