  • Judging by the answers here, it quite clearly means that someone should speak, rather than using ambiguous gestures.
  • This is just a shot in the dark, but I assume it suggests that the person has some very nasty things to say, but can't bring themselves to say it. Hence the action which is like saying, "There's so much that wants to come out of here at you, but I'll refrain and just do this to let you know how I verbally feel."
