Is it still illegal to sign someone else's name on a credit card transaction even when they are unable to do so and they explicitly ask you to do so on their behalf?

Not if you were given power of attorney either by them or by the courts. If you are acting as their agent (in case of a business credit card transaction), this is also allowable. In other cases it's forgery/fraud.
