For the blood clot to be detected it would need to be in the area of the heart or already went through the heart at the time of the echo cardiogram and stress test. If the clot formed after the test or if the clot traveled from the legs the tests would not have detected this.

Echocardiography can detect possible blood clots inside the heart. The isotope/nuclear stress test helps to measure blood flow to the heart, it creates images of the heart muscle.
