I'm sure they use it as a striking weapon, rather than skewering. If you watch the way they move, they are capable of sharp, powerful head movements. Moving like that in a school of fish would likely stun them, making them easier prey.

They use it to help them catch food fish. They go into a school of fish and thrash their bill around, shaking it and knocking some of the smaller fish. Then afterwards they can get the fish they stunned or injured. They also jump out of the water or 'breach' over a school of fish to stun them that way, too. It's all hunting to them.
