I would have to go with Josef Stalin. Even over Hitler. Although I would give him second place by only a nose. We will never know how many people really died because of Stalin. But if the estimates are correct, he far surpassed Hitler. And he did it to his own people. As far any leader from say the Middle Ages and back, I don't think we can apply our sense of ruthless to any of them. They were all pretty brutal by our standards. But to them and their enemies it was business as usual.
