Excuse my ignorance but have all Americans got post boxes outside their house or do some have letter boxes in their front door?

In some places there are mailboxes (like VA), but in Maryland, they have mail slots

I have a letter box? The little black box that hangs on the outside of my gate. I also have a mail slot, but since we have the gate, you can't get to the door.

it depends on the area some areas you are required to have a mail box accessable from the road / street for a postal delivery vehicle. some areas the mail is delivered by a postal delivery person on foot.

Some places have mail boxes, unlocked. Usually houses or trailers. Others have a large wall that houses many metal boxes with numbers on them, that can be opened with a key. Some houses have boxes on the door, or a mail slot.

It seems like older houses have mail slots, at least they did when I lived in New Jersey. Now it seems they are catering to the lazy postal workers so they don't have to leave their vehicles anymore.

There are both. I think in cities you see more letter boxes or mail slots in the door. In the suburbs and rural areas there are mostly mail boxes at the road.
