It's basically a game. I think it's a way of testing how gay or perverted someone is because it's usually near the crotch, so if you're caught, they punch you, if they look, you punch them.

Well, this game was created by the great Ben Williams in 2004. If you look at this hand gesture, you get beats and or hefty beats. If you get over 10 of these in a row, you get punched in the face. When Ben does it, you will usually be punched in the vein and or bone. It is closely related to "megs and beats", also created by Doctor Williams.

Call me silly, but if I knew it would get me punched, I guess I would just not do it. Since there is no picture any longer of any sign, I have zero idea.

Other people say it's some kind of a game. . I read in a "body language" book that in Australia the "okay sign" means: "Put it up Your ass!", "Asshole!" . And in Greece it means: "Fuck You!" (with a forward move) . So it's better not to hitchhike like that in these countries. :)
